About sleeping bag: I think the one good for AC will be good for Ladakh as well. There are some camps above 4000m where you need something warm to sleep in.
The direction of trekking should depend on your itinerary. If you fly to Leh, then it makes sense to start from Lamayuru and drive from Darcha to Ma*nali. Also if the trek is organized from Leh it is easier to arrange a start from Lamayuru.
Wherever you start the risk of AMS is high if you dont acclimatize properly. Leh is not a bad place to adapt to altitude before a trek since you can do lots of sightseeing along Indus valley where altitude does not vary.
